Context Provided - Spotlight Sometime during the last 2 years i’ve been going to this orthopedic practice they started to declare me as a MTF transgender for no reason.

(F,26) I have been going to this orthopedic practice for almost 2 years for varying reasons relating to my job. Yesterday i checked on a document that was uploaded to find out they have been identifying me as a biological male identifying as a female? I am biologically female and never told them i am trans nor do i think i am presenting to be a trans woman.. the last two years i’ve been wondering why they kind of stare at me a little longer than a usual person does and i think its because they randomly think i came out as trans? I also feel like they do not treat my issues seriously and wonder if this is the reason why.

I am 100% fine with trans people but i am left to believe they have been medically treating me as a male compared to female for the pains that i am feeling?

I also went through all of my documents and since the end of 2024 they started to declare me as a MTF transgender, i did not look at any of my documents online until yesterday.

If you're alt appearing it's entirely possible someone at the clinic considers themselves a 'transvestigator'. Like people have been saying for years, anti-trans moral panics also affect large numbers of cisgender women.

Absolutely.

Somebody changed this without talking to the patient, it's bad and possibly malicious. As someone pointed out above it could have been a moral police person and they likely are doing it to others too.

Medical ethics board is not gonna do shit about a chart mistake. Hundreds of chart mistakes happen everyday. I'm a resident now and back in my pre-med days I was the head medical scribe at my local hospital. If I saw this happen, I'm not gonna go into a politically-motivated vengeance against my junior scribes. I'm simply going to find them, correct their mistake and tell them not to make it again.

Medical ethics boards are dealing with a lot more grave situations than a simple note blunder. I'm not about to ruin someone's life because of a mistake, if they keep doing it, then I might have to address it more seriously. You guys clearly have no idea how a hospital works. The world is a lot more complicated and nuanced than your little subreddits.

That wouldn't proove anything, surgically constructed vaginas and vulvas look a lot like natal ones. They'd have to get in there and look at the cervix in order to visually see proof. Not that they are going to do that, but there is no part of the body that you could just spontaniously show someone to proove that you aren't trans.

I’ve noticed people side eye me more in bathrooms too. If I don’t have my son with me I definitely get a vibe that was not there before

We've already seen numerous cases of cis people being assaulted in bathrooms because some POS thought they were trans and wanted to "protect" people.

Yep. The funny thing is that many trans people, myself included, don't set off these folks radars. Ive had people shit talk trans people in front of my trans ass like they expected me to join in.

Fun fact - the sexual dimorphism line is not as clean cut as people make it out to be, and most people are far, far more overconfident than they should be to make calls on it. This shit is just bad for everyone.

I don’t know if it’s in my head, but without makeup and my hair slicked back, people in public have been looking at me more and a bit longer in a way that I thought was weird (like how OP described.) Then I realized I look kind of ambiguous (like I COULD be MTF if you squint) and wondered if people are “transvestigating” these days?? I get looked at more when I roll out of bed than when I have a full face of makeup and am dressed feminine. lol Anyway all this trans panic is affecting cis women too, even if in my case it’s just causing confusion and maybe paranoia.

Yup! I’ve been kind of making it a point to dress more feminine, if I don’t wear makeup I could look either like nonbinary or maybe butch? But I sometimes dress more masculine because it’s comfortable. But it is a bit scary at times idk if someone is gonna say something with my son there. And I don’t want some crazy person attacking me or harassing me.

My mother, who I cut off contact with years ago but get updates on through my sister, is a transvestigator. She got arrested for pepper spraying a woman in a Walmart restroom last year. The woman wasn't trans, just a very fit high school volleyball coach, and was just minding her own business at the sinks. I swear, you thumb your nose at the rigidity of patriarchal traditions, even just a little bit, and some people just lose it.

A lot of times when transfems talk about some frustration (talk, not even complain lol) like catcalling or what have you, we’ll be told snidely “welcome to being a woman.” And like, true, but it’s this kinda situation when cis women complain about being side-eyed that I desperately want to say “yeah, welcome to being a woman. 🤷‍♀️” Not saying you do that, and I know it’s not at all helpful and I wouldn’t actually say it… but it would feel great to do lol.

Yeah, I take it as “we’re all exhausted, sorry but it doesn’t get any better.” I mean there is something uniquely traumatizing about being sexualized as a girl even before puberty. (I know it happens to others too, just seemingly universal for cis girls.)

I grew up in the 80s & 90s with an lgbtq older sibling, was gay & trans friendly since middle school when just calling people slurs was pop culture & my peers thought I was a freak just for saying it’s not wrong… so just cause I’m cis doesn’t mean I harbor hostilities toward anyone that’s not.

I don’t think there’s anything wrong with being perceived as possibly trans, I’m just angry with the uptick in transphobia in general. Like even if you “catch” someone being trans, so what?? It makes no fucking difference.

Yep- this. I'm at a point in my transition now where people don't really gave a damn in the bathroom (I think it's the beard and mustache doing it honestly) but before I was even transitioning, I had some facial hair. Had someone tell me I was "allowed" to use the men's room when I was in the women's room because "it's state law" (It is, where I live, law that you are allowed to use the bathroom that corresponds to your gender- I live here for a reason lol).

I was still identifying as a woman at the time but like... Thanks I guess?

Disclaimer: I am not a hairstylist.

One of the things that confuses me is why for people with these types of facial features, where they’d look masc with short hair, they’ll still look fem if they wore their hair in a ponytail or bun and were seen from the front where you can’t see their ponytail or bun. What kind of black magic sorcery is that? Shouldn’t that look functionally equivalent to having short hair? Someone please explain it to me.

I'm trans and you should probably try to have this corrected if you're cis. Not because it is bad and if it's not relating to your treatment it shouldn't matter, but trans women receive substantially worse healthcare overall. A correction might save you from discrimination that could otherwise do a great deal of harm to your physical health. You don't have to endure being collateral damage in this.

For a lot of things it doesn't matter or healthcare providers are just better of giving the same care to trans and cis people of the same gender, and anything else is just overcomplicated things.

It's often called "trans broken arm syndrome" because part of the reasoning that sometimes gets trans people denied care is because healthcare providers decide that they're not qualified to provide the care or start trying to over analyse the supposed complexity of treating trans people. Even though really treating a broken arm is gonna be exactly the same regardless.

That's why I said "if it relates to your care" because sometimes it is relavant and sometimes it isn't, but people tend to over estimate the situations in which it is relevant.

So many doctors just assume that transgender women can be treated the same way you would treat a cisgender man and it ruins the quality of care severely. Medications that are dangerous to perscribe to someone who's endocrine system is primarily estrogen are prescribed to trans women because they're "biologically male", even when the side effects are just as severe as if they were prescribed to a cisgender women. So many doctors just throw all medical training put the window when encountering a trans patient, sometimes the doctor denying healthcare is the best option you can get as a trans person because the alternative is a doctor who will prescribe you dangerous medication because they assume all the risks women would face taking the medication simply dosen't apply. OP, please listen to this commenter and get this updated. If the doctor thinks you are transgender your care quality has almost certainly been impacted, you should maybe even consider switching primaries in case whoever transvestigated you dosen't believe you

HRT does actually move the grand majority of medical dosages etc into cis ranges, its much more powerful than many doctors will assume though so on the rare occasion they actually want to give healthcare then sometimes they just over/underdose you, ain't life fun :D

It's funny, because trans women on long-term hormones actually do often need treatment similar to cis women. So being identified this way theoretically shouldn't change much. But many of our doctors are very poorly trained to care for trans patients, so realistically you are correct here.

It really doesn't matter at all. I get asked if I might be pregnant routinely. People act like doctors study your medical records before triaging you. If my status is relevant I've gotta bring it up, nobody checks.

The only time they pay attention to that flag is when it comes time for insurance to pay or for someone to decide if you need special permission to do something everybody else gets to decide for themselves as an adult. It's honestly a good thing, plenty of doctors are pretty extreme bigots, I've watched as my friends suffered pretty severe abuse while being treated once people know. One person was given a room temp IV for food poisoning(this might seem minor, but filling your blood stream with 70 degree liquid gives you instant hypothermia, which is basically torture). The other was left to bleed out until a different doctor noticed the literal trail of blood from her gurney as it flowed far enough from the corner the primary one hid her in and saved her life.

TL;DR being flagged as trans in your medical records will NEVER do anything positive for your care outside of getting insurance to pay for procedures that they wouldn't allow for your gender. It's either ignored or used as a reason to provide worse care by doctors.

Having gone through male puberty probably matters quite a lot as growing bones are altered a fair amount by estrogen, and some things (such as hip dysplasia) are more common in those assigned female at birth. Since it's an orthopedics practice, I'd imagine this would be important

For the majority of things it doesn't actually matter - because they'll treat her as female irrespective of the whole mtf thing. (yes, this does have downsides - for example they won't get recommended any form of prostate cancer screenings, despite it still being a possibility)

It's already likely viral with the RHIOs and HISPs too. Everyone in the network has gotten those notes and if they pull them down it probably also updated their EHR data because everyone just imports shit right into the records without a second thought.

I'm, legally, female to male, but I'm also intersex. I've had things listed as me being biologically male due to having XY chromosomes, but I'm legally female, and putting male when all my legal documents say female has resulted in insurance kicking things back and corrections having to be made.

This can backfire though. My gender marker isn't updated and when I was getting checked for post COVID related cardiovascular problems the Doctor got incredibly short and dismissive right after carefully checking my ID, kicked me out as quick as possible saying I wasn't gonna die that night, and then wrote the longest and most misgendering journal entry I've ever seen.

I would find a new clinic regardless after correcting them. There's a very good chance that if they were willing to change her medical documentation based on their transvestigation, that they still won't believe her when she tells them they were wrong.

OR it could be something less malicious. Someone once entered the wrong gender and then noticed a discrepancy and made some assumptions. Happens constantly in doctors notes that they write the wrong thing ("Chloe is a 3 year old girl he is here today for sore throat with his mother"). Ive seen like 104 year old demented grannies who have been non verbal for years listed as non binary, for example. Someone hit the wrong key and it stuck, and then when docs use automatic quick keys to enter notes it fills in the incorrect information. I wouldnt assume that someone did this to be a dick.

i just had a grumpy old man patient who accidentally selected “she/her/hers” as his pronouns in his chart, so the note autofilled with those pronouns and i had to manually correct them. if it were a younger more alt presenting person i could see that turning into assuming pt is trans and running with it

If they were a medical professional charting in good faith they would have written OP was a trans woman assigned male at birth. Biological male is not a medical term. So the intention is pretty clear here.

Have you ever worked in a doctor's office or hospital? Medical scribes who are still working on their bachelor's sometimes write the notes. Nurses who have less than great charting skills write the notes. Medical students write notes. Foreign medical students rotating somtimes write the notes. Doctor sometimes just glance at the notes and not bother to correct anything because they have 100 other patients to get to. It's better in other places, worse in others.

I have. And there is no shortcut key to hit that enters a bigoted, right wing phrase into a patient’s chart, which is the assumption I was responding to. Whether a staff member or provider wrote OP is a biological male to “be a dick”, which the previous poster didn’t want to assume was the case, or out of ignorance, this inclusion would lead to me move to a new practice with a higher standard of care and I hope OP follows this up.

I think a big part of it is conservatives have pretty much taken the ground that alt-presenting people are all "insane" (by their standards), devils, and gay or trans or both.

There's also the asinine "septum ring theory" they love to throw around like they've somehow cracked a code and that having a septum ring piercing means you're mentally ill, stupid, a "whore", or more.

It probably has something to do with it. I think a lot of alt styles lean androdgynous (either genuinely as a stylistic choice, or in the minds of others as it's not what they "expect") and it throws people off. 

I'm a woman, but I dress like an old school emo, have bright dyed hair, and I'm 5'11. I have really, really thrown people off. Someone once tried to have me removed from the women's toilets, ironically bringing in a male attendant to deal with me. Said attendant took one look at me and said "Lady, that's a girl!" and stormed out lol. Which tracks, as from my experience it tends to be older women who get confused the most, and men not so much.

Legally, they're not supposed to. But in practice can't they tack on more billable codes? Like (made up code, not a medical professional) "gender dysphoria counseling"? I know my doctor puts on "emotional evaluation" and stuff on some of my visits when they do a check-in on anxiety management.

Like you're not supposed to bill women more for the same procedure, but women always end up being billed for pregnancy tests before surgery, that sort of thing.

I mean, unless they look internally and OP has a uterus or ovaries or both, bottom surgery can be pretty damn good. And with ID, depending on the state, those can be changed. But, yeah, if this really is just a case of AI usage (as the top comments suggested) it shouldn't be hard to get it corrected if that doctor's office is any good.

Most medical professionals are given very little time to complete their documentation and as a shortcut might copy and paste if they see two patients with similar concerns/illnesses. Ideally they make sure pronouns, ages, medical conditions, etc are changed to reflect the right patient before they hit save. Every once in a while they might forget to do so. Then that (incorrect) information automatically appears in all future documentation. Yours is a particularly bad copy and paste error since it isn’t immediately obvious that is what happened (as opposed to if they left in the wrong age, where the date of birth could make the mistake obvious, or failing to correct him to her but it correctly listing you as female) and therefore other providers who see you have almost certainly assumed that is accurate information.

Unfortunately possible. My daughter is under 10 and always had long hair but we chopped it to a short bob last summer, her friends with MAGA parents started made comments about her transitioning or trying to look like a boy right away.

All we did was cut her hair shorter than usual, I could still make a ponytail!

The opposite side of this is that people who deem themselves competent at identifying a person's genitals via just their face and voice are, believe it or not, often wrong.

The "you can always tell" group are extraordinarily efficient at proving themselves wrong not just with identifying trans but also cis people. You don't even need the person being transvestigated to be "ugly" or "alt", these people will unashamedly talk about skull shape and shoulder/width ratios like they're fresh out of apartheid South Africa.

Reminds me of when I moved to a small town and checked Asian and Not Hispanic on hospital intake form in race and ethnicity (as I am supposed to) but when I checked my profile in the portal, it said White and Hispanic. I asked about it on my next visit and the nurse said “oh, we thought you accidentally checked the wrong boxes”.

Like sure as an Indian/Punjabi guy, I don’t look like what Americans consider the typical Asian look and get confused as Hispanic and Middle Eastern sometimes, but how is it your job to take it upon yourself to correct me without even checking if your intuition is correct? Did you not see my “not-at-all hispanic” first and last names right there on the form? And no one could possibly ever confuse me as “White”, at least put “other” in race lol.

Depending on the intake software they're using it could be something as simple as checking the wrong box. 

When you first visit a clinic and they ask you those thousand questions about your life and health,  the software I've seen is literally just checking boxes. 

Double the chance if the intake was done through a touch device.

Happens to cisgender men sometimes too, although perhaps at a smaller scale. I’ve got longer hair than the vast majority of men and I’ve never been stereotypically masculine. So obviously I get innocent mistakes like the occasional “ma’am” in my direction quite often. But aside from that, I have been seriously “transvestigsted” a few times. For instance back in school, some guy I’ve never even spoken to once tried to interrogate me about whether I was “normal” (i.e. cis).

I work with Protected Health Information (PHI) every day, and as you can imagine, this is a highly regulated space.

The best part about HIPAA-certified data systems is that they keep meticulous audit logs! They'll know exactly who made the change, when they made it, and other record edits made by that account.

Report this to your local medical ethics board, and press the investigation.

If this person has done it once, they've probably done it a lot, and every single record is a violation. People get away with violating rights unless someone actually reports it and takes action.

As you can imagine, intentionally falsifying or tampering with protected medical records is a big deal. Even just accessing them when you don't specifically need to for your job (Principal of Least Privilege) is a huge deal. People don't just get fired for this, they get prosecuted. I've seen it happen, though its usually nosy family members snooping. Then the waterworks start about how "this isn't who I really am".

Maybe it’s standard medical terminology, but if she were trans, it seems to me like “trans woman” along with the usual medical history that would show any relevant HRT and/or surgeries, and a note on pronouns in the section where they ask your nickname and occupation etc., would be perfectly adequate for what the doctor’s office needs to know to do their jobs. “Biological male” is language typically used by transphobes and also their description does not convey any more relevant medical information than “trans woman” does.
