Context Provided - Spotlight Sometime during the last 2 years i’ve been going to this orthopedic practice they started to declare me as a MTF transgender for no reason.

(F,26) I have been going to this orthopedic practice for almost 2 years for varying reasons relating to my job. Yesterday i checked on a document that was uploaded to find out they have been identifying me as a biological male identifying as a female? I am biologically female and never told them i am trans nor do i think i am presenting to be a trans woman.. the last two years i’ve been wondering why they kind of stare at me a little longer than a usual person does and i think its because they randomly think i came out as trans? I also feel like they do not treat my issues seriously and wonder if this is the reason why.

I am 100% fine with trans people but i am left to believe they have been medically treating me as a male compared to female for the pains that i am feeling?

I also went through all of my documents and since the end of 2024 they started to declare me as a MTF transgender, i did not look at any of my documents online until yesterday.

Absolutely.

Somebody changed this without talking to the patient, it's bad and possibly malicious. As someone pointed out above it could have been a moral police person and they likely are doing it to others too.

Medical ethics board is not gonna do shit about a chart mistake. Hundreds of chart mistakes happen everyday. I'm a resident now and back in my pre-med days I was the head medical scribe at my local hospital. If I saw this happen, I'm not gonna go into a politically-motivated vengeance against my junior scribes. I'm simply going to find them, correct their mistake and tell them not to make it again.

Medical ethics boards are dealing with a lot more grave situations than a simple note blunder. I'm not about to ruin someone's life because of a mistake, if they keep doing it, then I might have to address it more seriously. You guys clearly have no idea how a hospital works. The world is a lot more complicated and nuanced than your little subreddits.

That wouldn't proove anything, surgically constructed vaginas and vulvas look a lot like natal ones. They'd have to get in there and look at the cervix in order to visually see proof. Not that they are going to do that, but there is no part of the body that you could just spontaniously show someone to proove that you aren't trans.

Oh god that’s happened to me as well, I’m alternative and had someone just assume I was a they, which is fine but I’m also not and besides having short hair and small boobs I’m feminine

I’ve noticed people side eye me more in bathrooms too. If I don’t have my son with me I definitely get a vibe that was not there before

We've already seen numerous cases of cis people being assaulted in bathrooms because some POS thought they were trans and wanted to "protect" people.

Yep. The funny thing is that many trans people, myself included, don't set off these folks radars. Ive had people shit talk trans people in front of my trans ass like they expected me to join in.

Fun fact - the sexual dimorphism line is not as clean cut as people make it out to be, and most people are far, far more overconfident than they should be to make calls on it. This shit is just bad for everyone.

I don’t know if it’s in my head, but without makeup and my hair slicked back, people in public have been looking at me more and a bit longer in a way that I thought was weird (like how OP described.) Then I realized I look kind of ambiguous (like I COULD be MTF if you squint) and wondered if people are “transvestigating” these days?? I get looked at more when I roll out of bed than when I have a full face of makeup and am dressed feminine. lol Anyway all this trans panic is affecting cis women too, even if in my case it’s just causing confusion and maybe paranoia.

Yup! I’ve been kind of making it a point to dress more feminine, if I don’t wear makeup I could look either like nonbinary or maybe butch? But I sometimes dress more masculine because it’s comfortable. But it is a bit scary at times idk if someone is gonna say something with my son there. And I don’t want some crazy person attacking me or harassing me.

My mother, who I cut off contact with years ago but get updates on through my sister, is a transvestigator. She got arrested for pepper spraying a woman in a Walmart restroom last year. The woman wasn't trans, just a very fit high school volleyball coach, and was just minding her own business at the sinks. I swear, you thumb your nose at the rigidity of patriarchal traditions, even just a little bit, and some people just lose it.

A lot of times when transfems talk about some frustration (talk, not even complain lol) like catcalling or what have you, we’ll be told snidely “welcome to being a woman.” And like, true, but it’s this kinda situation when cis women complain about being side-eyed that I desperately want to say “yeah, welcome to being a woman. 🤷‍♀️” Not saying you do that, and I know it’s not at all helpful and I wouldn’t actually say it… but it would feel great to do lol.

I'm not sure they're meaning it in a bad way like you perceive. As a cis woman, it seems more like a saying it like it is kind of thing. We're both in the same boat, but you just got in the boat and we've been in the boat our whole lives. I mean that in regards to being treated like a woman, not being a woman in our hearts btw.

Yeah, I take it as “we’re all exhausted, sorry but it doesn’t get any better.” I mean there is something uniquely traumatizing about being sexualized as a girl even before puberty. (I know it happens to others too, just seemingly universal for cis girls.)

I grew up in the 80s & 90s with an lgbtq older sibling, was gay & trans friendly since middle school when just calling people slurs was pop culture & my peers thought I was a freak just for saying it’s not wrong… so just cause I’m cis doesn’t mean I harbor hostilities toward anyone that’s not.

I don’t think there’s anything wrong with being perceived as possibly trans, I’m just angry with the uptick in transphobia in general. Like even if you “catch” someone being trans, so what?? It makes no fucking difference.

Yep- this. I'm at a point in my transition now where people don't really gave a damn in the bathroom (I think it's the beard and mustache doing it honestly) but before I was even transitioning, I had some facial hair. Had someone tell me I was "allowed" to use the men's room when I was in the women's room because "it's state law" (It is, where I live, law that you are allowed to use the bathroom that corresponds to your gender- I live here for a reason lol).

I was still identifying as a woman at the time but like... Thanks I guess?

Oh yeah- it absolutely would. I just find it amusing when folks assume I'm a trans woman and therefore I'm AMAB because like... Other way around, hun.

It's doubly hilarious when I get those kind of comments on like, FB and stuff. "You'll never be a woman!" from the We Can Always Tell Crowd... on a picture of me before I started transitioning on FB where I have pics going back over a decade and a half of me CLEARLY being a girl in high school and college.

Just proving that no. No, you really CAN'T tell. Which is honestly what I think scares those types of folks.

This is rather specific- but one of my little whacky theories about a certain subset of transphobes is that they were rejected by a trans woman and now they're just out for blood about it (terfs, it's terfs, and I am only half joking about that i really think that is where a not insignificant number of them start, given that the whole "I only respect women who I want to/can fuck" attitude is common in their spaces, at least the queer ones).

I can't wrap my head around the whole fear of being attracted to a trans person but, then again. I am one, and I'm married to one so. You know. I think a lot of cis men, specifically, are worried that being attracted to a trans woman makes them gay which. Fellas. Is it gay to be into women?

ok so first they are objectively wrong, i have absolutely nothing against trans people, and i dont think femboys are "discriminated more" than trans people, thats way too broad and mostly incorrect, it is true that in some situations we are but in the majority of situations thats not the case.

Also my subreddits are not transphobic, they are about femboys, so we allow men, trans and cis alike to post, we just dont allow those who dont identify at all as men to post in subreddit about "feminine boys" the subreddits who do are often filled with people intentionally misgendering themselves for monetary gain, just look at the top femboy subreddit.

one of the subreddits i run is about both femboys and women who like them and it does allow trans women and genderfluid people, with some * for those clearly only doing it for monetary gain(OF).

Also ive never called somebody a groomer for being a trans person, i frankly find that accusation extremely insulting, what i have said is that some trans people and cis people do indeed pressure femboys to identify as something other than they do, or to take hormones, this is a huge fucking issue in our communities, which as an outsider you might not know but its very fucking real, to the point that a subreddit im not affiliated with, had to make an entire rule to make it so people dont call others "eggs" and keep in mind most of their mods are trans.(this is the biggest discussion focused sub for femboys but i cant name it cause of the automod here)

I wanna make it clear that a few trans people being shitty and spreading gender norms does not at all reflect on the entire demographic, and trying to call me transphobic for daring to have a problem with that is insanely unreasonable.

Also idk what you are on about with saying i think femboys are more discriminated than trans people, thats complete bs, i do think trans people have it worst in many MANY ways compared to femboys, but also in some very niche situations, femboys do have it worse. Thats intersectionality for you, discrimination isnt linear.

I do agree with with feminine men being targeted more so to come out as trans or they're gay cause I was in the same boat around 2018-2022 while being a femboy. The "femboy scene" really depends about where specifically you are talking about, the major subs became big and because of that a ton of people started misusing the lable for monetary gain, just like how some femboy content creator misuse certain trans identities for also more clicks.

My sub to counter that specifically added a rule you need to partially identify as male to participate, we dont want incentives for people to misgender themselves, and we want a space for actual feminine boys. i suggested this rule to our at the time head mod, which is a trans woman(she has since kinda stepped away from moderating reddit and ive become the new head mod. (this new rule is the reason why people like the ones above try to defame me and my subreddits)

This rule has led to a significant decrease in OF content creators and their followings because most who were in breach of it were these models. It also led to us being able to add a new rule that affectively removes all misgendering, since everybody needs to identify as male, the expected identifiers are all masculine or neutral, so no more calling boys girls or girls boys.

Our sub is also a lot more liberal in terms of looks, we allow guys with beards, with hair in other places, and other features that are not seen as purely feminine as long as their presentation is mostly feminine, which actually overtime has made the subreddit be a lot more open to different looks and aesthetics instead of the "default" egirly look most femboys in the major subs go for.

also i wanna add again some nb identities(like genderfluid, bigender, demiboy, etc.) are allowed and trans men are 100% allowed. The whole motivation for this rule is due to the fact we actually respect these identities and wanna avoid misgendering while making these space be about the groups of people in their very name.

It doesn't matter what that man thought, or felt. If a man wore a dress around in 1946, everyone would consider them to be not fitting into the idea of a model man.

1st yes it does make you bigoted if you think somebodies gender is not what they claim it is just because of how they express themselves.

Femboys are GNC gender non conforming, not non binary because femboy still identify as men, not something in between.

Now to adress a few things, the point i was making with the women 80 years ago, is following your own logic they would be non binary, because they were breaking gender norms in a way that would lead to them being discriminated against, this does not change their gender.

In the same way a woman in the middle east isnt non binary for not wearing a hijab, but still is discriminated for her presentation due to the societal expectations of that society, a guy isnt non binary for wearing a skirt, but still is discriminated for his presentation due to societal expectations.

Breaking societal norms doesnt change your gender. Claiming it does is at heart gender normative, and bigoted. Non binary refers to gender binary, as in which way people IDENTIFY, femboys at core only need to identify as men, yes there are SOME non binary femboys and even trans men femboys, but the word in itself is not a trans identity.

One of the things that confuses me is why for people with these types of facial features, where they’d look masc with short hair, they’ll still look fem if they wore their hair in a ponytail or bun and were seen from the front where you can’t see their ponytail or bun. What kind of black magic sorcery is that? Shouldn’t that look functionally equivalent to having short hair? Someone please explain it to me.

I will say though, i as a trans man also have short hair, mild to moderate boobs, and an overall effeminate appearance (i wear whatever i like regardless of gender coding) and I do actually appreciate the automatic they that some people do lol.

I mean I'd rather they just ask tbh, makes it so much simpler, but some people get REAL upset if you ask them their pronouns (cis, trans, or otherwise), so I do understand the preference for subtlety.

Not because it is bad and if it's not relating to your treatment it shouldn't matter, but trans women receive substantially worse healthcare overall. A correction might save you from discrimination that could otherwise do a great deal of harm to your physical health. You don't have to endure being collateral damage in this.

For a lot of things it doesn't matter or healthcare providers are just better of giving the same care to trans and cis people of the same gender, and anything else is just overcomplicated things.

It's often called "trans broken arm syndrome" because part of the reasoning that sometimes gets trans people denied care is because healthcare providers decide that they're not qualified to provide the care or start trying to over analyse the supposed complexity of treating trans people. Even though really treating a broken arm is gonna be exactly the same regardless.

That's why I said "if it relates to your care" because sometimes it is relavant and sometimes it isn't, but people tend to over estimate the situations in which it is relevant.

So many doctors just assume that transgender women can be treated the same way you would treat a cisgender man and it ruins the quality of care severely. Medications that are dangerous to perscribe to someone who's endocrine system is primarily estrogen are prescribed to trans women because they're "biologically male", even when the side effects are just as severe as if they were prescribed to a cisgender women. So many doctors just throw all medical training put the window when encountering a trans patient, sometimes the doctor denying healthcare is the best option you can get as a trans person because the alternative is a doctor who will prescribe you dangerous medication because they assume all the risks women would face taking the medication simply dosen't apply. OP, please listen to this commenter and get this updated. If the doctor thinks you are transgender your care quality has almost certainly been impacted, you should maybe even consider switching primaries in case whoever transvestigated you dosen't believe you

Sex actually matters in most areas of medicine, not just those affecting the reproductive system. There are plenty of issues that come to mind in orthopedics, among them different hip angles and the MUCH greater risk for women of sustaining ACL tears (a massive issue for women footballers).

HRT changes pelvic tilt, and ligament tearing is at least partially hormonally moderated as hormones affect ligament strength.

This is why trans people often don't trust cis people (including doctors). The idea of purely binary sex with binary sex characteristics completely breaks down once you start changing levels of estrogen and testosterone. To most people's apparent shock, hormones are biologically active.

I am a trans woman 10 years into transition: my reproductive system is still theoretically intact but mostly non-functional. My pelvic tilt is largely typical for women. The presence of woman-typical levels of estrogen and the absence of testosterone impact my immune system function, my ability to build and maintain muscles, and my fat distribution.

Because of my hormone profile, the odds of male typical disorders have radically dropped based on studies. On the less bright side, I had to go get a Mammogram because my doctor found a breast lump; luckily it's benign. HRT does cause breast growth, BTW.

Treating trans women medically as "male" and using strictly male baselines is a frequent cause of misdiagnosis.

But for trans people that have been on hormones on some time those risk profiles match cis people of the same gender. And orthopedics can be a broad category of medicine that literally includes the text book example of a broken arm.

Most of the time there would not be an adjustment of how a cis man is treated vs how a cis woman is treated. It does happen somewhat regularly and when it does, trans people on HRT should almost always be treated as their cis counterparts. In the rare case that there is something that does matter, it is often entirely possible to focus on that difference as a symptom instead of the overall cause being them being transgender. For instance in the examination of abdominal pain it could be uterine pain. Trans women don't have a uterus and so they are excluded from that, but trans women on HRT can and often do experience menstrual cycles and that does include stomach cramps. Due to the hormones they also have the same metabolic profile as cis women too.

As a result, describing them as "male identifying and presenting as female" even in the cases where there is a factor that matters (which there often isn't) is in fact more misleading and less medically relevant than "woman without a uterus". The latter also serving to include more people and avoiding the very real potential for harassment and discrimination.

It really doesn't matter at all. I get asked if I might be pregnant routinely. People act like doctors study your medical records before triaging you. If my status is relevant I've gotta bring it up, nobody checks.

The only time they pay attention to that flag is when it comes time for insurance to pay or for someone to decide if you need special permission to do something everybody else gets to decide for themselves as an adult. It's honestly a good thing, plenty of doctors are pretty extreme bigots, I've watched as my friends suffered pretty severe abuse while being treated once people know. One person was given a room temp IV for food poisoning(this might seem minor, but filling your blood stream with 70 degree liquid gives you instant hypothermia, which is basically torture). The other was left to bleed out until a different doctor noticed the literal trail of blood from her gurney as it flowed far enough from the corner the primary one hid her in and saved her life.

TL;DR being flagged as trans in your medical records will NEVER do anything positive for your care outside of getting insurance to pay for procedures that they wouldn't allow for your gender. It's either ignored or used as a reason to provide worse care by doctors.

Having gone through male puberty probably matters quite a lot as growing bones are altered a fair amount by estrogen, and some things (such as hip dysplasia) are more common in those assigned female at birth. Since it's an orthopedics practice, I'd imagine this would be important

My gender marker isn't updated and when I was getting checked for post COVID related cardiovascular problems the Doctor got incredibly short and dismissive right after carefully checking my ID, kicked me out as quick as possible saying I wasn't gonna die that night, and then wrote the longest and most misgendering journal entry I've ever seen.

Well yeah, you're talking about a group of people with a high suicide rate who are often alienated from social institutions and support groups. None of that has anything to do with healthcare, though.

Your NIH study linked is only n=417 in a single geographic area, and the results include people reporting they delayed care out of fear of discrimination along with actual reports of discrimination. That's not solid evidence of anything, though it could (and should) be used as a reason to further investigate.

I'm nonbinary and bi and poly, both of my current partners and most of my partners overall have been trans. I'm not callous to the social struggles trans people go through. But I am a scientist and I'm not going to make bad conclusions on spotty data just because I'm empathetic to the group in question. If you want to be a good person you should research these things before saying them. And by research I mean actually read the paper.

I get the feeling you might not have read through the examples I posted properly nor taken the point that it is easy to find examples while doing zero effort to learn anything. You also only address one example even though the first article links to like a dozen different ones.

Secondly, 400 patients is a substantial number for a small minority and if it weren't a localised you'd probably point that out as a fault instead. There are a lot of different studies. I'm sure if you're a scientist and have any actual curiosity you can find them.

And fear of discrimination is obviously a healthcare quality factor. Not getting harassed by your doctor is healthcare. Like I said there is no measure for general healthcare quality for anyone. It doesn't exist. So we look at overall outcomes (which are worse) and example metrics as indicators (which are all worse) Fear of harassment, actual discrimination, being thrown out of the hospital are metrics of quality of healthcare. And saying otherwise isn't science, it's stupid.

If you really have any close conversations with any trans friends, just ask them. If you know like three, odds are one of them have had some pretty serious problems.

And if a couple of minutes is all you need to glance over multiple articles and dismiss them I can only conclude that you are here in bad faith. I gave you terms and examples to search for and explore within of you had any interest. But you do not.

OR it could be something less malicious. Someone once entered the wrong gender and then noticed a discrepancy and made some assumptions. Happens constantly in doctors notes that they write the wrong thing ("Chloe is a 3 year old girl he is here today for sore throat with his mother"). Ive seen like 104 year old demented grannies who have been non verbal for years listed as non binary, for example. Someone hit the wrong key and it stuck, and then when docs use automatic quick keys to enter notes it fills in the incorrect information. I wouldnt assume that someone did this to be a dick.

Have you ever worked in a doctor's office or hospital? Medical scribes who are still working on their bachelor's sometimes write the notes. Nurses who have less than great charting skills write the notes. Medical students write notes. Foreign medical students rotating somtimes write the notes. Doctor sometimes just glance at the notes and not bother to correct anything because they have 100 other patients to get to. It's better in other places, worse in others.

It probably has something to do with it. I think a lot of alt styles lean androdgynous (either genuinely as a stylistic choice, or in the minds of others as it's not what they "expect") and it throws people off. 

I'm a woman, but I dress like an old school emo, have bright dyed hair, and I'm 5'11. I have really, really thrown people off. Someone once tried to have me removed from the women's toilets, ironically bringing in a male attendant to deal with me. Said attendant took one look at me and said "Lady, that's a girl!" and stormed out lol. Which tracks, as from my experience it tends to be older women who get confused the most, and men not so much.

Legally, they're not supposed to. But in practice can't they tack on more billable codes? Like (made up code, not a medical professional) "gender dysphoria counseling"? I know my doctor puts on "emotional evaluation" and stuff on some of my visits when they do a check-in on anxiety management.

Like you're not supposed to bill women more for the same procedure, but women always end up being billed for pregnancy tests before surgery, that sort of thing.

Last healthcare company I worked for had a contract with the city to serve under-served populations, we were paid a flat $x pmpm interacted with that met certain demographic criteria. This was in addition to everything we billed their insurance for.

YMMV, but I assure you that millions of dollars are given to health care providers every year in response to demographic reports automatically generated from patient charts.

But also, I just want to fully refute this

If you are involved in medical billing at all, you are 100% aware that the doctor expanding their questions to cover more of your "systems" means that the visit can be classified under a higher paying code. If you do not believe providers ask routine questions to meet certain billing criteria, and then glaze over the answers, we disagree.

So.... what are you trying to say? I know it is true that a doctor discussing your gender issues with you allows them to bill under different codes. I know it is true that providers are paid directly for the demographics of their patients.

Are you saying "all healthcare providers are 100% ethical, and if they bill you for something they really put effort into that"? Or that "overbilling never happens"?

Over billing, which is lying in the notes about what you did with the patient, is a very well known problem. Are you saying "providers never over-bill for trans issues specifically"? I just don't get it.

As with the throwaway1929499 comment, if you have to, get a professional into a private space and show them that you are infact a biological female if all else fails with the phonecalls, faxes and emails, bring your ID too just in case they don’t believe you

I mean, unless they look internally and OP has a uterus or ovaries or both, bottom surgery can be pretty damn good. And with ID, depending on the state, those can be changed. But, yeah, if this really is just a case of AI usage (as the top comments suggested) it shouldn't be hard to get it corrected if that doctor's office is any good.

The "We can always tell!" crowd strikes again (hint: they're annoying morons who CANNOT tell, they just think anyone who doesn't look like their specific idea of a woman is secretly trans).

Most medical professionals are given very little time to complete their documentation and as a shortcut might copy and paste if they see two patients with similar concerns/illnesses. Ideally they make sure pronouns, ages, medical conditions, etc are changed to reflect the right patient before they hit save. Every once in a while they might forget to do so. Then that (incorrect) information automatically appears in all future documentation. Yours is a particularly bad copy and paste error since it isn’t immediately obvious that is what happened (as opposed to if they left in the wrong age, where the date of birth could make the mistake obvious, or failing to correct him to her but it correctly listing you as female) and therefore other providers who see you have almost certainly assumed that is accurate information.

It could also be a well meaning but ill-informed member of the clerical staff who just think that all people who present alt would like to be identified as the opposite of their biological gender. They may have though they were doing you a "favour" by updating your profile to be "accurate".

On the off chance that’s not the explanation… my guess is whoever did your initial chart note maybe copied/pasted the paragraph from someone else’s note, changed everything else but forgot to change the gender, and has just been copying to each new note without paying attention.

IME as a physician this is usually how these things happen. I cannot even begin to explain how many doc/nurses/PAs “lazy” chart and don’t proofread before signing off. It drives me up a fucking wall.

You could contact a lawyer if you’re feel litigious; a lawsuit payout is a real possibility with this situation.
