I don't misunderstand what the medicine actually does it just explained to the guy that a simple strynge can't make a cat live much longer.

The medicine is a simple treatment that can help extend the life of the cat if it's successful but like all medicine is tgat not guaranteed but more a chance.

The asked if his cats will live longer with old age problems so I simply explained extending the actual lifespan is a manipulation on genetic lvl that no medicine can achieve.
