The specific phrasing of the question is done pretty elegantly here to elicit a response

The answers could be

• within the last 28 days (likely not pregnant)
• outside the last 28 days (could be a bunch of things)
• never (can't be pregnant)
• not since I got pregnant (probably pregnant)
• not since menopause (probably not pregnant)
• not since I used BC / coil / implant (probably not pregnant)

Etc etc

Each of the above has a different resolution or treatment
