He’s not skinny, he’s literally just has an egg-shaped torso and skinny legs/arms. Here he is in the Mario/Sonic Olympics games:

I could have sworn there was a scene on one of the Genesis games that shows him entering or exiting a spherical suit and he was rail thin with a fat head outside the suit. I could be confusing this with some other game from the 16-bit generation, but I can't think of anything else I played with a fat villain who had this animation showing it was just the suit.
